DESCRIPTION: The Lifestyle Assistant is responsible to provide a range of
individual, group and community activities according to residents’
individual assessed physical, social, cultural and spiritual needs and
preferences.
QUALIFICATIONS
EXPERIENCE
ESSENTIAL:  First aid certificate
 Experience in residential aged care
 Certificate IV Leisure and Lifestyle
DESIRABLE:  Bus license
CONDITIONS OF
EMPLOYMENT:
 Per EBA and Contract of Employment.
 Position subject to a successful Police Check, including a timely
re-application before 3 years. Employees of AdventCare must
notify the Facility Manager if there has been a change in police
record status at any time before the renewal date of police
check, particularly if there has been a conviction recorded that
would preclude work in Aged Care.
RESPONSIBLE TO:  Lifestyle Coordinator
REPORTING
REQUIREMENTS:
 Reports any concerns to the Lifestyle Coordinator
MAIN DUTIES:  Completes social / lifestyle profiles of new residents and
document in care plan for leisure, interests and activities in
consultation with the RN in Charge and Lifestyle Coordinator.
 Documents exceptions to the care plan in the progress notes
and report same to the RN in Charge or Care Supervisor.
 Evaluates the care plan in partnership with the
resident/representative and in consultation with the RN in
Charge/Lifestyle Coordinator.
 Undertakes programs as per activities program which caters for
individual, group and community activities including outings and
for those residents with special needs, such as, vision impaired,
challenging behaviour and frail immobile.
 Functions as an active part of the team
 Maintains resources for the activities program.
 Effective communication between residents/staff and families.
 Good time organisation skills.
 Follows up of Tell Us about it Forms related to the activities
program.
PROFESSIONAL
CONDUCT:
 A high standard of personal appearance and conduct is
expected of every employee, that is: clean, neat, tidy, punctual,
and respectful language and manner toward residents, visitors
and to each other.

 At NO TIME must information related to a resident/family or
staff member be discussed with anyone other than the relevant
staff member/care professional providing care/service.
 Adheres to the facility’s uniform policy
 Polite and courteous to staff, residents, families and visitors
 A flexible, and enthusiastic attitude toward undertaking a variety
of tasks, and demonstrated commitment to ongoing quality, with
a team approach is expected.
 Adheres to AdventCare Policies and Procedures.
 At all times respect residents’ privacy, dignity, confidentiality
and individuality.
OCCUPATIONAL HEALTH
& SAFETY:
 Participates in the risk management program/ infection control
and contributes to a clean, safe work environment to ensure
safety of residents/visitors, other staff and self.
 Reports immediately, any equipment or situation which is
hazardous, or has the potential to be a safety issue.
 Reports immediately any situation of or suspected to be elder
abuse to the RN in charge according to the organisation’s
policies and procedures.
 Reports immediately Missing Resident to the RN in charge
according to the organisation’s policies and procedures.
 Participates in problem solving processes to resolve OH&S
issues.
QUALITY IMPROVEMENT:  Provides competent care/service in accordance with the
organisation’s documented policies and procedures, legislative
requirements relevant to role.
 Completes a Tell Us about it Form when there is an identified
need to improve.
 Actively participates and contributes to Quality Activities which
promote quality improvement to care/service provided, such as,
follow up of Tell Us about it Forms, attends staff meetings,
reads staff notice boards / bulletins, ongoing education, internal
assessment and review of procedures, evaluation of new
products and equipment.
ONGOING EDUCATION:  Participates in in-service and continuing education, including
mandatory training:
– Fire & emergency procedures
– Manual handling
– Infection control
– Bullying & harassment
– SDA philosophy
– Customer Service
– Food Handlers
 Membership of relevant professional groups and or
associations or network groups (if applicable).
 Works toward annual training objectives following appraisal to
ensure skills and knowledge to fulfil role.

PERFORMANCE
OUTCOMES:
1. Each resident has a current social profile and care plan with
identified needs, goals and strategies to meet the goals for the
identified social needs. Exceptional reporting and evaluation in
partnership with the resident / representative is evident in
residents’ record.
2. Resources are available and maintained to provide activities as
planned for residents.
3. Prompt follow up of Tell Us about it Forms related to activities
program.
4. Effective communication.
5. Effective team member.
6. Maintains a safe environment.
PERFORMANCE REVIEW:  6 monthly, then annually by the Facility Manager/ or delegate, or
as requested by employer or employee.
